New patients must fill out a new patient packet and complete an intake
interview.
Intake takes place every Monday and Tuesday from 10 a.m. to 2 p.m. No
appointment is necessary. You may download the new patient packet on their website.

For families/households with more than 8 persons, add $5,140 for each additional person.
Many communities have clinics that charge little or nothing for dental care. They are usually run by nonprofits, community health centers, dental schools and local health departments, and they often rely on grants, donations and public funding. Some serve everyone on a sliding fee scale, while others focus on children, seniors or people without insurance. Every clinic sets its own rules, so call ahead and ask what you qualify for.
