This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free.

Sliding Fee Scale and Insurance:
Family Health Centers provides services to anyone, regardless of the ability to pay. FHC accepts most insurance plans. FHC uses a sliding fee scale to determine “discounts” based on your family size and income. The sliding-fee-scale determines how much you will be asked to pay on the day of your visit, and how much you may be billed afterward. FHC’s minimum fee for Dental Services is $30.00.

For families/households with more than 8 persons, add $5,140 for each additional person.
