An oral health workshop held at the agency on the second and fourth Thursdays of the month at 10:00 am will be required for anyone interested in calling for a dental appointment. At the time of the appointment, a dental hygiene kit will be given to the customer to better ensure prevention of future dental problems.
Documents needed: Photo ID for head of household, Social Security cards for all members of household, any medical cards, proof of income for all household members over 18 years old for the past 90 days. Additional documentation may be required.
Referral procedures :Call in days for dental assistance appointments are the second Thursday of each month. Call this phone number, 217-535-3120, beginning at 8:30 am to schedule an appointment for that Friday.
