Vision Care Services.Case management services include bus tokens formedical appointments, assistance in completing forms, and referrals foremergency food, clothing, social services, and obtaining an ID.Freetransportation to healthcare appointments at Family Health Centerclinics is provided to homeless patients, Mental Health, Dentalservices, and Case Management at selected sites only; coordinate withthe assigned case manager/social worker.
